The referred patient must use your name when scheduling their consultation appointment. They must complete their surgery within three months of their consultation. We will credit $500 to you for each referral.  Credits can be applied to any elective plastic surgery procedure over $2000.  Credits expire after one year.  Credits cannot be applied toward medical plastic surgery cases, previous procedures, injectables, co-pays, or the anesthesia or hospital charges for any elective procedure.
